Do you care for somebody in your life? We could help you access your hospital details online

It’s now simpler than ever for you to view some aspects of your hospital and specialist community medical information online, through a platform called MY CARE - this can be accessed using an app on your mobile phone or computer.

At our events, we can support you to set up your own MY CARE account, and if you care for someone who has capacity to make their own healthcare decisions, we can make sure you have access to view their hospital details too (please note that your cared for person must come along to the session to do this).

If this sounds like something which you would like, or you have any questions about viewing your hospital care details online, come along to one of our upcoming events - please make sure you bring your NHS number, and either your mobile phone or laptop along.

Essential Digital Skills Support

Along with Devon Communities Together, we recently offered free Digital Skills support to those with long term health conditions in rural and coastal areas of Devon. 

This means we provided one to one and group support to help increase your confidence with digital technology and ensure that learners can carry out the tasks required to support their daily life. Whether you want to know how to email, access healthcare information, order your shopping online, video call family, or any day-to-day internet and technology related issues, we supported you to do this.

Virtual wards

Previously, we have partnered with Devon Communities Together and other voluntary and community organisations in a pilot project to help support patients from Exeter hospital who require remote monitoring. The two week ‘virtual ward’ enabled clinicians to continue to monitor patients from the patient’s homes, checking oxygen levels for example, without them needing to be in hospital. Wellmoor provided support with the technology required to continue the monitoring, ensuring the patient feels comfortable with using it and that the right information is being communicated to the clinical teams.
