VIRTUAL WARDS

 
 

The virtual wards project was a partnership pilot project between Wellmoor and Devon Communities Together, as well as several VCSE organisations over Devon, to support those who are receiving remote monitoring in their homes. Patients from the Royal Devon University Hospitals, primarily in Exeter, who still required monitoring but did not need to be in hospital, were given devices such as an Apple Watch or Kardia ECG reader to use at home, with this information being transmitted through to the clinical team at the hospital via MyCare, the patient’s online record system.

 

Wellmoor’s primary role in the project was to provide support with setting up and using the digital devices, using the skills developed from our Digital Befreinding programme to ensure that the devices were communicating the correct information to the hospital and that the patient was confident in using them.

 

Overall during the project we carried out 29 digital support visits to 25 patients.
